Charitable Event Donation Request Form

We are happy to consider your request for a donation to your charity event.  Since you know the guests who will be attending, we ask that you be specific about what you are seeking.  Such as a gift certificate, a crystal, or an item from our Gift Shop.   Requests must be made a minimum of one month prior to your event.   If you have an annual event, do us a favor and submit your request in December.  Because we receive numerous requests each month, we ask your understanding if we cannot make a donation.
